Who is Legacy Health Services?

Welcome to Legacy Health Services We are a family of skilled nursing and assisted living facilities throughout Northern Ohio that operates on integrity, humor, passion, and relationships. By hiring the best and most qualified people, we provide our residents with the right care and services when and where they are needed.

How do I contact legacy employment services?

What is the legacy portal and how does it work?

The Legacy Portal gives providers and medical staff quick access to some of their most-used resources and tools, including Epic, PACS and other Citrix applications, news highlights, eDocTalk, and quick links to clinical tools, schedules, training, education and more.

How do I submit an application to Legacy Health?

All applications must be submitted through Legacy Health’s Electronic Application System. Resumes submitted via email, fax or mail will not be accepted in lieu of an electronic application. Legacy Health strives to make this site accessible to any and all users.

Records Requests

To request your occupational health record, complete and return this form. You may also use this form to have records forwarded to Legacy Employee Health Department from other providers.

CDC Adult Immunization Program

Legacy Employee Health maintains specialized expertise in current recommendations for adult and health care worker immunizations and provides over 20,000 shots each year. All current Legacy Health employees are eligible for free adult immunizations and may bring in their Legacy-insured dependents ages 14 and older for free immunizations.

Workforce Wellness

All Legacy Health personnel are eligible for basic wellness services at no cost.These include: nicotine replacement, first aid, vitals, weight, BMI, body fat percentage, health education resources, medication cards, feminine hygiene supplies, lactation rooms, over-the-counter medications and flu shots.

Covid-19 Vaccination Requirements

All candidates who accept a position with Legacy Health must have received the complete dosing regimen of one of the COVID-19 vaccines currently authorized by the U.S. Food and Drug Administration (FDA)
