Insurance for eligible employees is effective the first day of the month after 30 days of employment with GHS. At no cost to the employee, GHS provides life and AD&D insurance of 1 x annual base salary up to a maximum of $180,000 for the employee, $5,000 for the spouse, and $2,500 for each dependent child.
